BIRTHDAY PARTY RESERVATION PROCEDURES
(please read before printing forms at the bottom)
Thank you for choosing Xtreme Challenge as your Birthday Party destination. Your reservation is not complete until the deposit form, deposit, and this reservation form have been signed and received by Xtreme Challenge. This form must be used for ONLY Birthday Parties. This form and your deposit form are linked at the bottom of this page for printing. If you are booking a different type of event (e.g., youth group, on location, camp days, or corporate teambuilding), please refer to our website for the correct forms.
Although this reservation form may seem a bit lengthy (just a bit), please read all notes below to better understand our arena procedures. Our friendly reminders will ensure that your special event runs smoothly, happily, and joyfully.
#1: WAIVER FORMS:
Each guest must have their waiver form signed ONLY by their legal guardian. Other adults are NOT allowed to sign for children for whom they are not a legal guardian. When carpooling, please make sure all children have completed forms with them. No one is allowed to play without a completed form!
*Our waiver form is two pages in length - please make sure your guests have completed both sides. Waiver forms must be filled out and initialed in their entirety. The waivers cannot be modified or crossed out in any way or the participant will NOT be allowed to participate-NO EXCEPTIONS.

#2: ADULT WAIVER FORMS:
All adults entering the arena floor must complete a waiver form, regardless of whether they are joining in play-based activities or simply taking photos. Please make certain other adults with your group are aware of this requirement.

#3: ARRIVAL TIME:
Please arrive 15 minutes prior to your scheduled arena time, and remind your guests to arrive at that time as well. As a courtesy to parties in progress during your arrival, please do not arrive more than 15 minutes early. During this 15-minute period, our staff will check in your party guests, check their waiver forms, and distribute wristbands to all participants so that your party can begin on time.
#4: LIST OF GUESTS:
Remember to bring your list of "expected" guests, and give this to our coach or party host who will be checking in your guests with their waiver forms. Please do NOT email the list; instead please bring it with you. For safety reasons, Xtreme Challenge cannot allow children three and under to be on the arena floor for any reason. All infants and toddlers must remain in the spectator or party room area only.
#5: GUEST COUNT:
Please EMAIL (preferred) or call us no later than 24 hours prior to your event to give us the final count of expected guests including the age, gender, and any other special information for each guest. Your guest count needs to include anyone four and over that enters the arena floor. Siblings on the arena floor will count as guests. For children’s parties, ONLY the birthday parents are allowed to play for free (these spots are nontransferable and are reserved for birthday PARENTS only). Please note that your child’s birthday party will be customized based on your child’s age group.
#6: CASH OR CHARGE ONLY ACCEPTED for FINAL PAYMENT:
Your balance is due the day of the event. Only cash or credit card payments will be accepted.
#7: GRATUITIES:
Many parents ask us if it appropriate to tip their party staff. Our specially trained X-treme Coaches and staff are at your service to make your
event extra special -- and they love serving you! If you think they did a GREAT job, a gratuity is appreciated and graciously accepted by your
extraordinary party staff!
#8: PARTY ROOM:
We offer a party room area for an additional $38.00 for 30 minutes. If you would like to reserve this area, we do require that you reserve the area
when you originally schedule your event. Please note: NO outside food or drink is allowed in our lobby for cleanliness and safety reasons.
While you are out on the arena floor, our staff will set up your RESERVED party room area. For your party room section, we will provide one
tablecloth for the head table (CAKE TABLE) and, of course, we will clean the area after your party. Please remember to bring your own paper
plates, utensils, cups, matches, cake knife, candles, food, and drinks. You are welcome to bring additional table clothes if you would like to
theme your guests’ "dining" tables. For your convenience, we offer use of our freezer for your frozen birthday cake or ice cream and we offer
cold drinks for purchase.
#9: BRINGING FOOD INTO THE FACILITY:
We have partnered exclusively with Pizza Hut (other pizza companies are not allowed). You may view their menu and order online at www.pizzahut.com or call them at
(303) 799-6566. Pizza Hut is the only pizza that may be brought into the arena. HOWEVER, by request, you may bring in other outside
(non-pizza) food if you wish. We recommend a 24 hour pre-order time to make sure pizza is available and
arrives on time. We do not allow: coolers, crockpots, or skilllets in the facility and we ask that
you please refrain from bringing chips and small candies. All food brought into the facility must be consumed in YOUR pre-reserved party room
table area ONLY. PLEASE NO EXCEPTIONS!
#10: BRINGING DRINKS INTO THE FACILITY:
We DO NOT allow coolers inside the facility. If you wish to serve drinks, please transport them in a paper or plastic bag.
#11: NO PINATAS, OR SILLY STRING:
Please refer to our list of things to bring on the following page for a list of items we do not allow into our facility. These items are a safety hazard for our guests.
#12: DIRECTIONS, DON'T LET YOUR GUESTS LEAVE HOME WITHOUT THEM! WE ARE VERY HARD TO FIND, PLEASE REMIND YOUR GUESTS TO USE YOUR WRITTEN invitation DIRECTIONS to our facility!
For some unknown reason, GPS and Google maps do not accurately locate our facility!
