Participant Waiver and Release Form
The Waiver Release form must be signed by the child participant's LEGAL GUARDIAN ONLY. NOTE: Other adults are NOT allowed to sign for children for whom they are not a legal guardian. Legal guardian proof, may need to be presented, at Xtreme Challenges discretion/request. NO exceptions will be made for participants who do not have a form. Xtreme Challenge reserves the right to refuse admittance, for incomplete, or improperly filled out, and/or suspicially forged forms. To ensure that each guest brings a form, please print and include a clearly printed two-sided form with your invitations. Participants who arrive without their parentally COMPLETED and SIGNED waiver form will NOT be allowed to participate. Additionally, if any of the boxes are not initialed, or any part of the waiver is crossed out or modified, that participant's waiver will be considered invalid to participate. Please emphasize these WAIVER requirements to each of your guests’ parents via email or verbal REMINDER. We find many guests car-pool with other guests, and therefore don't have forms from THEIR PARENTS to participate.
Please help us, by being diligent and letting your guests PARENTS know they MUST bring the properly filled out WAIVER form to participate.
